Ministry launches Chronic Care Connect pilot and Jamaica Moves app in Spanish Town
The Ministry of Health and Wellness has introduced new digital tools aimed at helping Jamaicans take greater control of long-term conditions such as diabetes and hypertension.
Officials unveiled the Chronic Care Connect Pilot alongside the Jamaica Moves mobile application during a launch event in Spanish Town. Both initiatives pair technology with community-based care to widen access to services and encourage healthier lifestyles.
The free Jamaica Moves app, developed under the Health Systems Strengthening programme, allows users to record physical activity and track blood pressure, blood sugar, and weight from their smartphones. The Chronic Care Connect Pilot will enrol roughly 600 volunteer patients across six health centres over the next year.
Planners said the model links electronic health records, community health aides equipped with practical tools, and outreach within neighbourhoods. The wider approach also applies tailored social and behaviour-change strategies for different stakeholder groups, alongside advocacy, communication, and community mobilisation to strengthen patient confidence, awareness, and ownership of their care.
The launch drew support from the European Union in Jamaica, reflecting a shared view that a healthy population underpins a strong and resilient economy. Together with the Inter-American Development Bank, the European Union has committed J$1.8 billion toward initiatives that include new and renovated hospitals and health centres, more efficient health services, and expanded training for staff.
Authorities expect findings from the year-long pilot to shape future strategies as Jamaica broadens the use of digital tools in chronic disease management and public health delivery.
